What Sensitivity Actually Is
The crown of a tooth has enamel on the outside — hard, dense, and containing no nerve supply. Beneath it is dentine, which is different in an important way: it is threaded with thousands of microscopic tubules running inward toward the pulp, where the nerve lives.
When dentine is covered, those tubules are sealed off. When it is exposed, cold, heat, sweetness, acid and even air movement cause fluid within the tubules to shift, and that movement is registered by the nerve as a sharp, brief pain.
This explains the characteristic sensation: it is sudden, it is sharp, and when the trigger is removed it stops quickly. It is a mechanical signal rather than an injury.
The Test That Tells You Most: How Long Does It Last?
Before any of the causes below matter, this distinction is the one that determines urgency.

The Common Causes, and What Each One Needs
Gum recession
When gums recede, they expose the root surface. Roots have no enamel — they are covered by a thin layer called cementum that wears away readily, leaving dentine directly exposed. This is the most common cause of sensitivity along the gumline, and it is why sensitivity often appears in the thirties and forties rather than in youth.
Recession can be caused by gum disease, by brushing too hard, or simply by the natural thinning of tissue over time.
Brushing too hard, or with a hard brush
This is worth stating plainly because it is so common and so preventable: scrubbing hard with a firm brush wears notches into the tooth at the gumline and pushes the gum back. Plaque is a soft film. It requires contact and technique, not force. A soft brush used gently removes it entirely.
Acid erosion
Acid dissolves enamel directly. Frequent sources include citrus, sports and energy drinks, carbonated drinks including sparkling water, wine, and vinegar-based foods. Reflux and frequent vomiting expose teeth to stomach acid, which is considerably stronger.
Frequency matters more than quantity. Sipping one acidic drink across two hours is harder on enamel than drinking it in five minutes. And it is worth knowing that brushing immediately after acid exposure removes softened enamel — waiting about an hour, or rinsing with water first, is better.
Grinding and clenching

The part usually missed: rather than rinsing it away, smear a little onto the sensitive area with a clean fingertip last thing at night and leave it. Contact time is what makes it work. Give it two to four weeks before judging it.
- Switch to a soft brush and lighten your grip. Holding the brush like a pen rather than a fist is a surprisingly effective correction. Many electric brushes have a pressure sensor — it is worth paying attention to.
- Change how you consume acidic things, not necessarily whether. Drink them in one sitting rather than sipping. Rinse with water afterwards. Wait about an hour before brushing.
- Avoid whitening while sensitivity is active, and pause it if sensitivity develops mid-course.
- Note the pattern before your appointment. Which tooth, which trigger, how long it lasts. That information genuinely shortens the path to a diagnosis.
What Treatment in the Office Can Do
The approach depends entirely on the cause, which is why diagnosis comes first.
- Fluoride varnish applied to exposed areas — quick, and often effective for generalised sensitivity.
- Bonding or a filling to cover an exposed notch at the gumline.
- A night guard where grinding is the driver.
- Treating decay or replacing a failing filling, where that is the source.
- Adjusting the bite where a restoration sits slightly high.
- A gum graft, referred to a periodontist, for significant recession.
- Root canal treatment, where the nerve is irreversibly inflamed — the point at which sensitivity has become something else.
When Sensitivity Is Telling You Something More Serious
Call the office rather than waiting if you notice any of these:
- Pain that lingers well after the trigger has gone
- Pain that starts on its own, with no trigger
- Pain that wakes you at night
- Pain with heat that is eased by cold
- Pain on biting one particular tooth
- Any swelling of the gum or face, or a pimple-like bump on the gum
- Sensitivity that appears suddenly and severely in one tooth
Facial swelling with fever, or difficulty swallowing, is an emergency and needs immediate care.
